Membership Form

I understand that upon completing the membership form below my credit union (or organization) will be billed directly. After our annual membership fee of $199.95(US) is paid, I will receive an access notification that will enable me and my committee members to have complete internet access to the ICUT program facility.  I also understand that one membership permits all staff and volunteers from our credit union to have access to this facility for one full year and that our membership can be canceled at any time with written notification.
